The four stages, explained
STRAW+10 (Stages of Reproductive Aging Workshop, 2011 update) is the gold standard clinicians use. It puts cycle pattern at the center because hormones swing too much for blood tests to reliably stage you under 45.
Late Reproductive Stage
- Cycles still regular but subtly shorter
- Possible early symptoms: PMS worsening, sleep changes
- Fertility declining but possible
Early Perimenopause
- Cycle length varies by 7+ days from your usual
- Hot flushes and night sweats can begin
- Mood, sleep and brain fog often start here
Late Perimenopause
- Skipped periods of 60+ days
- Hot flushes and sleep disruption usually peak here
- Final period typically within 1–3 years
Postmenopause
- 12+ months since your last period
- Vasomotor symptoms gradually ease for most women
- Bone, heart and vaginal tissue need ongoing attention
