Early Signs of Perimenopause

The classic image of perimenopause is hot flashes and missed periods. But the earliest signs usually arrive years before that — and they're often dismissed as stress, age, or 'just me.'

The earliest signs

  • Cycle length changes — even 2–3 days shorter or longer is a signal
  • PMS getting worse — heavier mood swings, more bloating
  • Sleep changes — waking at 2–4am, harder to get back to sleep
  • New anxiety — especially if you've never had it
  • Brain blips — walking into a room, forgetting names mid-sentence
  • Heavier or lighter periods
  • New food sensitivities — bloating from things you used to tolerate
  • Weight creeping up with no obvious cause
  • Energy crashes in the afternoon

When does early perimenopause start?

Most commonly late 30s to early 40s. Anyone can experience earlier onset; if you have premature ovarian insufficiency (POI) — symptoms under 40 — it warrants medical assessment.

What to do early

  • Start tracking now — even a year of data is gold when you talk to a clinician
  • Strength training — this is your bone and muscle insurance for 30 years
  • Protein audit — most women are short by 20–40g daily
  • Sleep hygiene — protect this fiercely
  • Find a clinician who actually understands midlife women's health
