Perimenopause Age Range: When Does It Start and End?

When does perimenopause start, when does it end, and what's normal at each age — from late 30s to early 50s.

Perimenopause Age Range

Perimenopause can start as early as the late 30s and as late as the late 40s. The average is around age 45. It ends at menopause — defined as 12 months after the final period — with the average age of menopause being 51 in the US and UK.

Decade-by-decade

Late 30s

Early peri is possible. PMS may worsen, cycles may shorten slightly, sleep may shift. Tracking now pays huge dividends later.

Early 40s

Most common start window. Cycle changes become more obvious. Mood, sleep, and energy fluctuations begin.

Mid-to-late 40s

Mid-perimenopause for most. Irregular periods become the norm. Hot flashes and night sweats often appear. Brain fog and weight changes peak. This is also the most common window for starting HRT.

Early 50s

Late perimenopause or menopause. Periods may stop. Symptoms often peak in the 1–2 years right before menopause.

Post 51 (average menopause age)

Post-menopause. Many vasomotor symptoms ease over time. Long-term focus shifts to bone, heart, and muscle health.

Early menopause and POI

Menopause under 45 is called early menopause; under 40 is premature ovarian insufficiency (POI). Both warrant medical assessment — HRT is usually recommended for bone, brain, and cardiovascular protection in these cases.
